Module content learning objectives and structure

Designed by Monash University, School of Nursing and Midwifery, each of the modules contains a specific set of learning objectives and has been carefully laid out for optimum learning purposes.

The structural content of each of the modules has been designed to reflect the needs of the general practice workplace and the requirements of Medicare Item 10997 that is linked to the care planning activity undertaken by the general practitioner in consultation with other members of the health care team.

The learning objectives have been based upon a template used in each module that introduces chronic disease pathophysiology; clinical manifestations; management and care planning; health monitoring; supportive interventions based on current best evidence; and evaluation of chronic disease management and care.

Comprehensive glossary complete with list of acronyms

A comprehensive glossary complete with list of acronyms has been provided in every module of the e-learning training package for easy access to a range of information.

Complete reference list

A complete reference list that identifies all resources used in the e-learning training package is available in each of the modules. This reference list has been provided in APA format because it is a commonly used reference system, particularly for nursing students. We encourage use of the reference list as a valuable resource to further research and learning.

Recognition Assessment

The e-learning professional training program to support Medicare Item10997 includes a Recognition Assessment Tool that has received a noted tick by the National Quality Council.

The Recognition Assessment Tool has been developed to support the recognition assessment for the chronic disease self-management skill set from the Community Services Training Package CHC08.

The package meets competency standards associated with the following units from CSTP CHC08 -:

CHCICS407A Support positive lifestyle
CHCICS408A Provide support to people with chronic disease

Individuals who successfully complete either one or all of the modules contained within the ‘e-learning training package to support Medicare Item 10997’ can use their Certificate of Completion to apply for recognised prior learning or credit transfers.

It is important to note that the Recognition Assessment Tool that has been developed provides for partial recognition against each of the three units listed above. It is therefore up to individual registered training organisations to assess competency against Certificates of Completion dependent upon application from individuals.

Introduction to skill sets

It is important to note that most skill sets included in the CHC08 Community Services Training Package are designed to build on a relevant qualification to enable the candidate to undertake work in the area addressed by the skill set.

Skill sets do not replace qualifications as the foundation for undertaking work in the community sector. They enable a qualified worker to move laterally into work areas addressed by the skill set or to broaden their skill base in relation to the services they provide.

This skill set has been endorsed by industry as appropriate for people who hold a community services or health qualification at Certificate IV level or higher or commensurate industry skills as evaluated through recognition of prior learning processes.

It provides a set of skills required to assist clients with self-management of chronic disease

These units provide credit towards a range of qualifications in the community sector.

Successful completion of the modules attract 5 Continuing Nurse Education (CNE) points each for modules 1, 4 and 8 (Foundation, Coronary Heart Disease and Kidney Disease) and 3 CNE points each for modules 2,3, 5, 6 and 7 as part of RCNA’s Life Long Learning Program (3LP).

The modules are consistent with the Competency Standards for Nurses in General Practice (ANF 2006) and the Australian Nursing and Midwifery Council competency standards.

CD ROM version of the e-learning training package to support Medicare Item 10997

A CD ROM version of the ‘e-Learning training package to support Medicare Item 10997’ has been produced as an optional delivery method for those who may experience difficulty accessing the modules on line. This version contains the complete set of modules from the package, and is presented in a Dual CD ROM set.

People who opt for this delivery method should be aware of the limitations imposed on gaining Continuing Nurse Education points and Certificates of Completion when using the CD ROM. In order to maintain the integrity of the e-learning training package and the awarding of CNE points and Certificates of Completion upon successful completion, people who use this version need to undertake the following:

Upon completion of each module mandatory self-assessment task, note or print your answers.

Keep a record of these answers upon completion of your selected module, access the e-learning training package online (see http:www.3lp.rcna.org.au/10997) and log-in using your login and password

Go to the mandatory self-assessment tasks in the module and record your answers

You will be advised if you have reached the required 80% pass rate and be eligible for a Certificate of Completion and Continuing Nurse Education points (if a practice nurse)

Submit your answers once you have achieved the results you seek. You will then be able to generate a printable Certificate of Completion. If you are a member of RCNA, CNE points will be automatically attributed to your portfolio.
